Registration Process and Forms

NOTE: The registration process and forms have changed.  Please follow the new procedures carefully.

STEP 1:  Christian Life Academy Registration Form

This is a 2 page form.  Page 1 is for personal and contact information and also requires the parent/guardian signature.  Page 2 has fee, family cap, discount, deposit, volunteer requirements, and uniform responsibility information.  Circle the appropriate fee to pay from the fee table (at the top of the form) on page 2.

This form must be completed for all student athletes and for each sport that they are participating in.  One form per student athlete is required.

STEP 2:  MSHSL Eligibility Brochure and Annual Sports Health Questionnaire

This is an 8 page form.  Pages 1, 7, and 8 must be completed.  Pages 2 – 6 are for information purposes only.  The entire document may be turned in but at the minimum pages 1, 7, and 8 are required.

This form must be completed for all students in grades 7 through 12 and only needs to be done once for the entire school year.  Therefore if your child has participated in an earlier sport this school year at CLA then this form will already be on file and a copy of this form is not needed.

STEP 3:  Sports Qualifying Physical Examination Clearance Form (SQPE)

This is a 4 page form.  A copy of page 1 is all that is required to be turned in.  The complete document can be kept for your own records.

All students in grades 7 – 12 must have a physical to be able to participate in athletics at CLA.  For proof of a physical this form must be completed and typically only needs to be done once every three years.  Therefore if your child has had a physical as a 7th grader then they do not need to have a physical again until they are a 10th grader.  If your child has participated in an earlier sport this school year at CLA then this form will already be on file and a copy of page one of the SQPE is not needed.

STEP 4:  Fees and Deposits

CLA families will have their FACTS account billed for the appropriate participation fee which is on the back of the registration form.  If you are eligible for a discount or meet the family cap limit be sure to also submit the appropriate form for this.

In the event that any CLA family forfeits all or a portion of their uniform or volunteer deposit then their FACTS account will billed for the appropriate amount.

Important notice to CLA families with FACTS accounts.  CLA policy states that if you are more than 60 days late on the payment of FACTS incidental expenses that your FACTS account will be suspended and you will be required to pay for all school expenses on a cash only basis until all FACTS expenses are paid in full.  Be sure your FACTS account expenses are up to date.

Co-oping and other participating families will need to pay fees and deposits via check, money order, or cash.  Deposit checks from a previous sport season in the same school year can be used.  Deposit checks will be destroyed at the end of the sports year.

STEP 5:  New Home School and Other Participating Families Only

If you are a new home school or other participating family desiring to participate in the CLA athletic program please contact the Athletic Director, Denise Larson, at 651-463-4545 as soon as possible.  Depending on the sport your student will participate in and the grade of your student will also determine what additional MSHSL and CLA forms that will need to be completed to ensure your student’s eligibility to participate.

STEP 6:  Complete All Forms and Submit at One Time

Your first opportunity to hand in all of the required forms and documents will be the night of the Registration and Information Meeting.   That night the coaches will be available to take your registration material.  You must turn in all required documents at one time and they must be completed in full.  No documents will be accepted by the coaches if there are any missing or partial documents or partially completed documents or documents missing signatures.

After the night of the Registration and Information Meeting coaches will not accept any forms from the students or parents.  If you do not get your child registered the night of the Registration and Information Meeting you will then need to bring all of the required documents in to the high school office and turn them in to the A. D.   Do not attempt to turn in any forms to your coach at a practice or any other time.
